Because of how iOS works. The security parameters of the Wi-Fi network you are on — the encryption type, for instance — are only available to apps that hold location permission. The app reads the network parameters, not your coordinates: nothing about your position is stored or sent anywhere. If you refuse the permission, the Wi-Fi checks simply become unavailable and the rest of the audit runs as normal.
iOS deliberately does not let third-party apps read certain settings — whether Find My is on, whether your iCloud backup is encrypted, whether a SIM PIN is set. No app can read them; anyone claiming otherwise is guessing. So those checks are a guided checklist: you answer once, the answer is saved on the device and counts towards the score, and you can change it whenever the setting changes.
The rating is the weighted share of checks that passed, expressed from 0 to 100. Weights exist because the risks are not equal: Wi-Fi encryption carries a weight of 1.5, a SIM PIN and Lockdown Mode carry 0.5, and everything else carries 1.0. Without weights, turning on a minor setting would move the score as much as fixing a genuinely exposed network.

An address may be missing for ordinary reasons: the breach has not been published or catalogued, the company has not disclosed it yet, or your address was not part of the leaked set. Not being found is good news, but it is not proof that your address is safe.
Touch and hold an empty area of the Home Screen until the icons start to jiggle, tap the plus button in the corner, find Device Trust Scan in the list, choose the small or medium size and tap “Add Widget”. The widget shows your current rating and how many items are left to fix, and it updates after each audit.
Because it is not an antivirus, and on iOS an antivirus in the usual sense cannot exist: apps run in a sandbox and cannot inspect other apps or system files. What this app does instead is audit the settings that are actually within your control — encryption, permissions, passwords, breaches — and show you exactly what to change.
